Yes, it’s a villain undercut both in this movie and in Kingsley’s latter-day return in Shang-Chi, but wasted potential still requires some quality for fans to be upset over.
It's that frenzied landscape that allows The Mandarin, the shadowy figurehead leader of the Ten Rings played by Sir Ben Kingsley, to thrive. The standout scene where he threatens the Mandarin by giving away his home address stands up there with one of MCU’s most badass moments and – whisper it – is a touch cooler than his "I am Iron Man" mic drop.

Upon a revisit for its 10th anniversary, it’s clear that it’s overflowing with style, charm, and charisma – something that Marvel’s post-Endgame era has been struggling to find. Not quite a success (though the $1.2 billion box office would beg to differ) and not quite a failure, the Shane Black-directed threequel feels like the poster child for Marvel’s forgotten middle-of-the-road mediocrity. The giant electromagnet above the surgeon's heads may have been an invention created specifically by Stark to aid a previously unwilling surgeon to take the risk in removing the metallic shards.Then there’s Iron Man 3. Why not utilize it to remove the shrapnel if he could? It would be logical for Stark to create a means of activating and utilizing the Extremis process in conjunction with an excellent surgical team because even if they made a mistake removing the shrapnel, a slight activation of the Extremis technology would repair any damage long enough for any surgeon to do their work.
Since he was able to reverse-engineer the technology and effects of Extremis from Pepper, he would have both the technology to create the process as well as reverse it. He had been told it would be impossible to remove the fragments no matter how skilled the surgeons were, none were willing to risk it. If Stark had been able to remove the fragments before now, he would have done so rather than risk death aggravating his potential heart injury several times by driving his body to its limits. It is not definitively resolved but we can make several suppositions:
